If it is decided that an appointment will be scheduled, you will be emailed a link and login information for a portal that will contain all relevant intake paperwork, office policies, and HIPPA consents. These documents will need to be completed prior to your first scheduled appointment.
Rates and Insurance
Dr. Stabler is on the following insurance panels: United Healthcare, Blue Cross Blue Shield, Aetna, and Humana. New clients are advised to contact their insurance company prior to their first appointment to check benefits. If clients are covered by a different insurance provider, it is advised that clients check the out-of-network benefits with their insurance carrier. Upon request, receipts with procedure/treatment codes will be provided for clients to self-submit for out-of-network coverage reimbursement. Flex-spending accounts may also be used to cover costs of treatment. Fee for service is also available. Payments are expected at the time of service and can be made with cash, credit card, or check. Cancellations made within 24 hours of the appointment time are charged the full session amount.
